Introduces include/asm-i386/mach-*/mach_dma_map.h. Fixes a potential
problem (when CONFIG_SWIOTLB=n) with the previously submitted swiotlb.h
merge patch. Consolidates things so there are less differences to
x86-64's implementation as well as to minimize the number of things that
need to live in include/asm-i386/mach-*/mach_dma_map.h.
