[Bug 1626] 2.6.32 Dom0 crashes after lv* commands

I see a very similar thing if I do quick logout + login from/to iSCSI targets
to which I have two connections (using multipath):

It works ok if I give the system a couple of seconds between logout and login
but if I do it quickly I get those:

I am able to continue working on the system but it usually leaves me with
defunct scsi or multipath devices that I can't remove from the system anymore.
