A mere MoS policy, that may apply only to [[en:]] anyway,
shouldn't be enforced by the software in the default option.
Not that it's that simple, given your second point:

But I have seen a couple occasions where links in headers were good.
(Rare, but existent.) They should not break -- and they haven't broken,
but we should still be sensitive to that possibility when designing defaults.
