Chapter 14 isn't clear about the relation between smtp_accept_max_per_host and
smtp_accept_reserve, more precisely whether the latter provides exception from
the former. The documentation references them (along with smtp_accept_max)
together, but doesn't provide info on the relation of smtp_accept_max_per_host
and the reserve.
