To follow up and close this item, I have determined that the problem was an
incorrect entry in the "blacklist_from" list which caused a 'hit' when it
shouldn't have. Thus, an email might come in which was in the whitelist but in
going through the white/black lists, it would also be found in the blacklist.
Once the incorrect entry was fixed (several) things returned to normal.
Whether this suggests that maybe there should be tighter checking on these
entries I will leave open.
