The basic maildir structure could be used for mail formats other than
RFC 822.
If a maildir is used for 822-compliant messages, the first line will
always be Return-Path.
If a maildir is used for 822-compliant messages with a stored envelope
recipient address, the second line will always be Delivered-To.

If you're using Return-Path and Delivered-To, you should say so in your
documentation. You can assume that they show up on top.
